Person Centred Planning (PCP) is a whole approach and way of working with people with learning disabilities that puts the individual at the centre and ensures that the support and services they get are those they want as well as need.

Life Path Trust is committed to helping people achieve their personal goals. The person centred planning work helps people find out what their goals are and how they are going to achieve them.

There are a range of tools that our staff have learnt to help people express their views. These are called; Paths, Maps and Essential Life Styles. All of the tools use pictures and graphics to help people understand the issues. People with learning disability who have done some planning work have told us:

"I liked the drawing pictures of my favorite things"

"Speak freely and draw"

"Enjoyed the day"

"Pauline enjoyed telling everyone about her dreams"

All staff are trained in person centred working and those supporting people with person centred planning are trained facilitators.
